We aim to support people in increase their independence, maximising their potential, and moving on to independent accommodation. We encouraged clients to sustain achieved goals and take control of their lives whilst utilising and participating in their community
What We Do?
We provide supported accommodation with intensive housing management (housing-related support) and specialist support and advice for individuals with diverse strengths, needs, and aspirations to develop confidence and resilience through a wide range of opportunities. We support our clients in creating a personalised support/action plan that recognises their strengths, skills, and needs. Clients are encouraged to take positive risks and supported through the move-on i.e. the process of getting their private rented accommodation to live independently.
Who Do We Work With?
We work with a single vulnerable adult with an identified support need such as:
We provide low to medium supported housing for adults between 21 to 65 years old. Our accommodations are for a maximum of 24 months and are a mixture of shared properties, en-suite and studio in a house. All our accommodations are fully furnished. The client has their private bedroom and shares communal area in shared accommodation, i.e. bathroom, the kitchen, and lounge.
We offer support in the

- Assist with rebuilding and sustaining daily living and social skills
- Assistance to manage challenging behaviour and complex needs
- Engagement in meaningful activities: assistance with finding employment, enrolling in education, volunteering, and accessing training.
- Assistance to claim entitled and manage benefits
- Assistance to participate in community activities
- Assistance with motivation and taking responsibility
- Programme of in-house and external activities to minimise boredom and isolation
- Supervision to prevent exploitation from others to vulnerable clients
- Support and befriending to reduce social isolation when living alone
- Support to register and attend appointments with primary care services
What does the service offer?
Housing Support Officer access individual client need, and then agree to a support plan. The plan sets out what needs each client has and how the Housing Support Officer will work with the client to meet the needs. Next, the individual client and Housing Support Officer agree to the support plan by using Outcomes star to focus on support planning.
The individual client set his or her own personalised goals, and the plan stated how Housing Support Officer would be working with the individual client to achieve the set goals. The client meets with the Housing Support Officer regularly to achieve the set goals during key working sessions. Changes to clients’ needs or goals are updated on an ongoing basis and are reviewed every 3 months, but this depends on individual needs. In addition, there are weekly house meetings where Housing Support Officer meets with the clients in a project to discuss any tenant-related issues or personal issues.

- Maintaining socially acceptable levels of personal hygiene
- Preparing shopping list, going shopping etc

- Signposting to external agencies and support groups
- Befriending & chatting with clients
- Complying with terms of the tenancy
- Dealing with disputes with neighbours or other tenants
- Emotional difficulties & coping with change (informal counselling)
- Resettlement & preparation for move on accommodation
