Who Can Refer?
We accept referrals from voluntary (charity/ homeless organisations) and statutory providers (government agencies) only. Please note: Self-referrals are not accepted.
Referrals
Important information before making a referral:
Clients must have no active illegal drug/alcohol use and must be willing to engage with support services.
Optimism Supported Housing is unable to assist those with a history of arson attacks, and current self-harm.
Optimism Supported Housing is unable to assist those assessed as high or very high risk of harm.
Before a referral can be made, please ensure the client has valid proof of ID e.g. passport, birth certificate, or drivers licence.
Referral Process
The referring organisation will complete referral forms, along with a
- Completed Application form
- Current / relevant Risk and Needs assessment and
- Consent forms authorising the sharing of information with third party.
Optimism Supported Housing will assess the referral application in line with the OSH criteria. The individual client will be contacted with the offer of an assessment appointment.
Waiting List
Optimism Supported Housing has a waiting list: When there are no vacancies, eligible clients are placed on the waiting list and when vacancy/vacancies become available clients on the waiting list are informed.
Level of Support
Clients of Optimism Supported Housing receive low to medium level support. We will also refer clients to other specialist services, as and when appropriate or needed.
